Well, Yes and No. Karma system is supposed to encourage not engaging some factions, but from what we've seen from back in 2016 this was map specific. Some maps allowed engagement of bot yours and enemy factions, some would punish engaging both. We really are lacking most info about Karma at this point to be able to say much.

Probably, yes. At the very least the main storyline quest will have cut-scenes and an actual narrative.

I doubt this. Play-style is based on who is playing, not the faction you chose. There may be differences in the story and some quests because of the faction you chose, but essentially everything else is the same for both. Factions will start with different skills, but i don't think that will affect any play-style, only the weapons they are a bit more proficient with.
4. Karma system that punishes you for killing people in your own faction.

5. VOIP that allows you to play together with people in your own faction.
VOIP implementation is still kind of on the fence. Radios will use VOIP for sure and those will enable you to communicate with anyone on that frequency, they won't have to necessarily be from your faction. If there will be area VOIP without the need for radios is still what isn't confirmed as the DEVs are reluctant to add it due to concerns over the immersion.
6. A shift away from PvP and closer to PvE.
This i doubt as well. PVP is always going to be an option for those who want it and don't mind the Karma penalty. There will be an increase on other areas to focus, like the main story questline, unlocking maps, unlocking traders, etc but PVP isn't going to be limited.
7. EFT Questline which acts as an actual story on top of current quests.
Yes. It will be tied to other aspects of the game such as what maps and traders you have unlocked as well.
8. Plenty more AI enemies and factions to fight such as Russian Army, Cultists, UN Army, etc.
You pretty much named all the missing factions. Not sure if 3 is 'plenty' but you are correct on that one. Must be said, however, that those are all AI factions and the only playable factions are the Scavs and PMCs.
9. Less PMCs to fight but they are more dangerous.
Raid PMC count isn't likely to decrease. If anything it'll only increase. They are aiming for +/-40 for streets of Tarkov for example.
10. More realistic and punishing medical system which includes systems like carrying wounded teammates.
Unsure about the 'realistic' part. While they are planning a sort of rework of the medical system, they did mention the Medical system isn't something that they won't go too in depth with, and i doubt we'll shift away from some of the current implementations.
11. Map-to-Map travel based on the specific extract you take within a given map.
No. This is the biggest misunderstanding people usually have. There won't be map to map travel with the exception of accessing Labs and Reserve. Raids gamemode will be exactly what we currently have. The UI is due for an update in the future but in essence you will always choose a raid map to go into and when you extract you get back to the menu.
The FreeRoam game mode, which is what most people get confused with, is going to be all 10 raid maps and some of the space inbetween them stitched into a single map with no loading zones between them (according to the DEVs). This will only be unlocked after completing the main quest and 'escaping' Tarkov.
12. Being able to find loot and kill enemies is "harder".
It will get progressively harder the further along the game you go. Early maps will be easier and have less good loot. As you unlock more raid maps the harder the AI will be and the loot will also be better.
Another couple of things:
First location will be a sort of 'tutorial' location to allow new players to get a grasp of the basics. Unsure if it will be repeatable or if you are able to get loot from this location as it will technically not have other players in it.
There will be 3 character slots per account. They will not share progression or the stash/hideout. They can be different factions.
Global dynamic loot manager will make items more or less rare depending on a 'global' pool, as well as shift loot spawn locations based on how visited a location is. So more visited locations will see diminishing loot quality spawns while less traveled locations will see an increase.
Traders will need to be unlocked as they are tied to a specific map, which also needs to be unlocked via exploration/quest/extracting the previous map.
A third gamemode will be called 'Arena' which will be a deathmatch type of game mode featuring 1v1, 2v2, 3v3, etc in smaller maps with shorter timers. This will be unlocked somewhere during gameplay (With a side quest line most likely).
